Job Summary

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team as a Medicines Optimisation Specialist. As a key member of our pharmacy team, you will play a vital role in ensuring that our patients receive the best possible care and treatment.

Main Responsibilities
  • Work as part of a multi-disciplinary team to clinically assess and treat patients using extensive experience and in-depth knowledge.
  • Develop and implement pharmacy-led medicines optimisation strategies to improve patient outcomes and reduce healthcare costs.
  • Collaborate with general practice teams and other healthcare professionals to ensure seamless care coordination and patient safety.
  • Lead and promote medicines optimisation initiatives to improve patient care and reduce waste.
  • Contribute to the development and implementation of clinical guidelines and policies to ensure best practice.
  • Provide education and training to healthcare professionals on medicines optimisation and patient safety.
  • Participate in quality improvement initiatives to enhance patient care and reduce healthcare costs.
Requirements
  • Minimum of 2 years post-qualification experience in a primary care or hospital setting.
  • Good knowledge of NHS medicines optimisation and patient safety principles.
  • Excellent communication and interpersonal skills to work effectively with patients, carers, and healthcare professionals.
  • Ability to work independently and as part of a team to achieve shared goals.
  • Flexibility to work across multiple sites and travel to meet with colleagues.
  • Commitment to ongoing professional development and continuing education.
