About the Role

We are seeking an experienced Care Coordinator to join our dedicated team at Apollo Home Healthcare Ltd. As a skilled Care Coordinator, you will play a key role in the continuous delivery of care to service users.

Key Responsibilities

  • Manage a caseload of care packages and understand the varied nature and fast-pace of the role.
  • Apply your communication and relationship building skills to effectively manage rota's for your care teams.
  • Respond efficiently to any day-to-day changes and act as the first point of contact for service users, their families and other healthcare professionals.
  • Work well under pressure, using your initiative and thinking on your feet.

Requirements

  • Experience in care coordination, preferably in a similar setting.
  • Excellent communication and interpersonal skills.
  • Ability to work well under pressure and think on your feet.
  • Valid driving license and access to a vehicle.
  • Commitment to supporting the out of hours on call service.

  • Competitive salary between £28,290 - £33,290 pa, including payment for on-call.
