Role Overview

As a Specialist Respiratory Community Nurse, you will be responsible for delivering high-quality patient care in the community setting. This role requires independent travel across the county, adhering to strict time schedules. A valid UK driving licence and access to a vehicle are essential, as public transport is not a viable option for this position.

Key Responsibilities
  • Plan, implement, and assess tailored specialist care programs for patients, utilizing evidence-based practices and clinical reasoning.
  • Encourage self-management among patients and actively engage with support groups.
  • Maintain professional and legal accountability for all aspects of your work.
  • Efficiently manage your caseload, prioritizing clinical needs and optimizing time usage.
  • Conduct assessments and reviews for patients on oxygen therapy in the community, including blood gas monitoring in accordance with established guidelines.
  • Supervise and guide assistant staff, delegating tasks as necessary.
  • Provide education, support, and counseling to enhance understanding of respiratory conditions for patients, families, and involved agencies.
  • Collaborate with the multidisciplinary team (MDT) to ensure a coordinated service and help prevent unnecessary hospital admissions.
  • Participate in Community Respiratory Team meetings, MDTs, and other relevant gatherings as required.
