Role Overview:
We are seeking a skilled and dedicated Social Worker to join our Children with Disabilities Team in Newcastle. As a Social Worker, you will play a vital role in assessing the needs of children and young people with disabilities, and developing personalized support plans to help them achieve their full potential.
Key Responsibilities:
- Conduct thorough assessments of children's needs, identifying areas of strength and weakness
- Develop and implement individualized support plans, in collaboration with families and other professionals
- Provide guidance and support to families, helping them to access resources and services
- Work closely with other agencies and professionals to ensure seamless delivery of services
- Monitor and evaluate the effectiveness of support plans, making adjustments as needed
Requirements:
To be successful in this role, you will need:
- A degree in Social Work or a related field
- Experience working with children and young people with disabilities
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and analytical skills
