About Jubilee Medical Centre

Jubilee Medical Centre is a leading healthcare provider seeking a highly motivated Clinical Pharmacist to join our team. As a Network Pharmacist, you will play a crucial role in ensuring the safe and effective use of medications, providing pharmaceutical care and expertise to patients, and collaborating with healthcare professionals to deliver exceptional patient outcomes.

Main Responsibilities

  • Conduct comprehensive medication reviews and assessments to identify and resolve medication-related problems for patients.
  • Collaborate with physicians and other healthcare professionals to develop and implement personalized medication therapy plans.
  • Provide patient education on medication use, potential side effects, and drug interactions.
  • Monitor and adjust medication therapy based on patient response and laboratory values.
  • Conduct medication reconciliation and identify discrepancies to ensure accurate and up-to-date medication lists.
  • Participate in multidisciplinary meetings to discuss patient care and optimize treatment outcomes.
  • Provide education sessions for healthcare professionals and patient groups.
  • Stay up-to-date with current literature and guidelines to provide evidence-based pharmaceutical care.

Requirements

  • Master of Pharmacy degree from an accredited institution.
  • Active pharmacy registration with the General Pharmaceutical Council.
  • Good knowledge of pharmacology, therapeutics, and medication management.
  • Excellent communication and interpersonal skills.
  • Ability to work well in a fast-paced environment and handle multiple priorities.
  • Non-medical Prescribing qualification desirable or an interest in working towards obtaining the qualification.
  • Experience of working in a team and using Windows-based computer systems.
  • Experience of the EMIS clinical system.
