A Collaborative Team Environment

Our Adult Social Care service consists of highly skilled and supportive multidisciplinary teams. We foster a culture of positivity and inclusivity, ensuring that our social care teams operate cohesively to provide optimal support for our residents.

We work in close partnership with health professionals and other stakeholders to prevent unnecessary hospital admissions and facilitate timely discharges, ensuring that individuals can return home safely and swiftly.

Our Early Intervention and rehabilitation services are designed to help individuals remain in their homes, and we conduct regular multidisciplinary forums to ensure that health and social care services are integrated and accessible.

Understanding the challenges of working in Adult Social Care, we prioritize the well-being of our staff. Regular supervision and training opportunities are provided to support professional growth and development, and we value feedback from our social care staff to enhance our service delivery.

Key Responsibilities and Professional Development
  • Manage a diverse caseload, effectively organizing and prioritizing tasks with ongoing supervision and support.
  • Collaborate within a multidisciplinary team to connect individuals and their families with community resources and social networks.
  • Work alongside colleagues to ensure that individuals receive appropriate care at the right time and place.
  • Employ a strengths-based approach to empower individuals to achieve their best possible quality of life while reducing reliance on long-term care.
  • Uphold high performance standards across the service.
  • Operate within allocated resources as per our governance framework.
  • Utilize evidence-based practices in service delivery.
  • Engage actively in personal supervision and appraisal in line with departmental policies.
  • Take ownership of personal learning, including mandatory and recommended training.
  • Lead on specific projects as required.

Ideal Candidate Profile

We are seeking qualified Social Workers registered with Social Work England, with a minimum of two years of post-qualification experience in statutory social services, typically within a local authority context. Candidates should demonstrate a strong understanding of best practices and relevant legislation, coupled with a motivated and professional attitude.
