Our Adult Social Care service is comprised of skilled and supportive multidisciplinary teams. We foster a culture of collaboration and accessibility, ensuring that our social care teams work in unison to provide the best possible support for our residents.

We collaborate closely with health professionals and other partners to prevent unnecessary hospitalizations and facilitate timely returns home when care is required.

Our focus on Early Intervention and rehabilitation services is designed to help individuals remain in their homes for as long as possible.

Regular multidisciplinary forums ensure that our residents have straightforward access to coordinated health and social care services, providing holistic support tailored to their needs.

Recognizing the challenges of working in Adult Social Care, we prioritize the well-being of our staff. Regular supervision and training opportunities are provided to support professional growth and development. We value feedback from our social care staff and actively seek their input on enhancing our services.

Your Role Will Involve...
  • Managing a caseload effectively, prioritizing tasks with guidance and supervision.
  • Collaborating within a multidisciplinary team to connect individuals and their families to community resources and social networks.
  • Working alongside colleagues to ensure timely and appropriate care delivery.
  • Adopting a strengths-based approach to empower individuals to live their best lives while reducing reliance on long-term care.
  • Upholding high performance standards across the service.
  • Operating within allocated resources as outlined in our delegation framework.
  • Utilizing evidence-based practices in your work.
  • Engaging actively in your own supervision and appraisal processes.
  • Taking responsibility for your professional development, including mandatory training.
  • Leading specific projects as needed.

Ideal Candidate Profile...

We are seeking qualified Social Workers registered with Social Work England, possessing a minimum of two years of post-qualifying experience in statutory social services, typically within a local authority context.
