Cera Care is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in ensuring the delivery of high-quality care services to our clients.
Key Responsibilities:
- Manage the delivery of care services to clients, ensuring that care workers are matched to clients based on their needs and preferences.
- Conduct care assessments and reviews to ensure that clients receive the most appropriate care.
- Motivate and support care staff to deliver high-quality care services.
- Develop and maintain relationships with clients, their families, and care staff.
- Ensure compliance with relevant regulations and standards, including CQC/CI/CIW.
Requirements:
- Relevant qualifications, such as NVQ/SVQ 2 or 3.
- Registration with the SSSC (Scotland) or SCW (Wales).
- At least 2 years of experience working as a care coordinator for a domiciliary care agency.
- Knowledge of care delivery and management principles.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
