Position: Care Support Specialist
Service: Housing & Community Support Services (HACSS)
Responsibilities:
The primary objective of this role is to deliver a compassionate, efficient, and effective 24/7 support service to individuals utilizing The Action Group's services. This includes aiding individuals in developing their skills by supervising and engaging in various support activities that enrich their opportunities and promote choice. The key responsibilities for this position are outlined below:
Specific Responsibilities
Implement the TAG Key Worker system, which involves organizing support plans and case reviews for individuals we assist, ensuring effective communication between staff, and maintaining accurate records. Foster the physical and emotional wellbeing of the individuals we support at all times. Build meaningful and trusting relationships with the individuals we assist. Encourage independence in daily living activities, providing support only when necessary. Promote hobbies, interests, and community involvement, including organizing outings and activities. Provide personal care assistance as required. Support individuals in enhancing their communication skills and advocate for self-advocacy. Liaise with external organizations on behalf of the individuals we support regarding benefits, education, and employment. Collaborate with family members and caregivers to ensure their involvement in the individual's life. Stay informed about local services and resources available to those we support. Participate flexibly in various shift patterns to meet the needs of the individuals we assist. Occasionally cover for short-term absences of other support specialists.General Responsibilities
Adhere to TAG's Mission, Aims & Objectives, and current Business Plan. Manage your time effectively and maintain organized documentation. Communicate promptly and clearly with colleagues and individuals we support. Stay updated on and comply with all relevant TAG policies and procedures. Promote and respect individual rights and responsibilities at all times. Maintain confidentiality in all aspects of the role. Contribute to your own Continuous Professional Development (CPD) and support TAG in maintaining its Investors In People standard.
