Purpose of Job:

To work as a key member of the clinical multi-disciplinary team at St Andrews Hospice, contributing to the effective and efficient delivery of holistic care for patients with life-limiting illnesses, their families, and significant others.

Main Responsibilities:
  1. Assess, plan, implement, and evaluate patient care within a holistic, patient-centred approach, continually reviewing and updating nursing care plans and documentation.
  2. Observe and report accurately any changes in a patient's condition, physically or emotionally, and liaise with the Clinical Lead Nurse or Lead Nurse as necessary.
  3. Maintain the highest clinical standards and quality of care, contributing to and sustaining a caring and friendly environment.
  4. Manage the unit for the span of duty as the Nurse in Charge when required, following a successful probationary/preceptorship period.
  5. Deliver care prescribed with optimum attention to detail, being the key worker to designated families.
  6. Negotiate changes in care with patients, families, and other professionals.
  7. Promote effective communication amongst all disciplines, patients, and carers, communicating sensitive information in a considerate and confidential manner.
  8. Work flexibly alongside patients and families, acknowledging their expertise in their care.
  9. Liaise with the patient's doctor, other health professionals, and members of the multi-professional team prior to and during admission to the Adult Inpatient unit.
  10. Coordinate referrals to other specialist agencies according to the assessed need of the patient.
  11. Develop skills and knowledge in all aspects of palliative care, working as an autonomous clinical expert.
  12. Review and evaluate documentation used by the nursing team, ensuring accuracy, legibility, and appropriateness.
  13. Communicate via patient records, including SystmOne, and maintain, store, and retrieve appropriate records.
  14. Initiate and take responsibility for reporting and investigating incidents and complaints.
  15. Remain aware of current Health & Safety legislation in relation to the environment.
  16. Work effectively within a team of nurses and other healthcare professionals.
  17. Promote high staff morale by inspiring a shared vision and common goal.
  18. Develop and maintain key working relationships with the multi-professional team.
  19. Conduct oneself in a professional manner, being a role model and leading the staff.
  20. Provide support and direction to nursing assistants and volunteers, and act as an associate mentor for students.
  21. Be involved in the collection of data and evidence to support audit and quality outcomes.
  22. Undertake the updating of policies and procedures in line with government proposals.
  23. Take responsibility for own continued professional development.
  24. Manage own workload and that of others, ensuring appropriate prioritisation and delegation.
  25. Proactively assess own development needs and seek out development opportunities.
  26. Partake in external and in-service training, developing expertise in palliative care.
  27. Attend and participate in mandatory training and relevant meetings on a regular basis.
Management of People:
  1. Direct: When in charge of a shift, all staff and volunteers during that span of duty.
  2. Indirect: Other clinical staff and volunteers when a situation arises that requires clinical direction.
Contacts & Relationships:
  1. Regular contact with staff, volunteers, and managers to ensure optimum service levels.
  2. Frequent contact with distressed patients and relatives.
  3. Act as a point of reference for peers and subordinate staff for general departmental queries.
  4. Follow processes to satisfy the requirements of the Hospice policies and procedures.
  5. Continually seek to improve systems within the Hospice.
  6. Participate in regular staff meetings.
  7. Provide an internal consultancy/specialist advisory role.
  8. Maintain clear communications with inter and multi-disciplinary teams.
  9. Attend and participate in external palliative care planning meetings.

Frequent exposure to bodily fluids, including blood.
