Key Responsibilities:
- Engage with individuals in a personalized plan of support, working collaboratively to achieve their goals.
- Provide practical information and advice on issues such as budgeting, welfare benefits, employment, training, relationships, and health issues.
- Complete Safety Management Plans with individuals, ensuring a positive and collaborative risk management approach.
- Refer to and/or make appointments with other organizations according to individual needs.
- Work collaboratively with individuals, empowering them to make decisions about their support.
- Work with a range of other professionals, such as social workers and landlords.
Essential Criteria:
- Enhanced DBS (adults and children)
- Driving license
- Knowledge of mental health and risk
