Job Summary

We are seeking a highly skilled Community Mental Health Practitioner to join our CYP crisis and intensive support team. The successful candidate will work closely with Specialist Mental Health Practitioners and the wider MDT to provide high-quality clinical services to children and their families.

Main Duties
  • Deliver a high-quality clinical service to children and their families, including specialist assessment, care planning, and treatment interventions.
  • Manage a complex caseload, including assessing patient needs, developing and implementing care plans, and evaluating treatment outcomes.
  • Work collaboratively with the MDT to provide comprehensive care and support to children and families.
  • Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
  • Contribute to the identification of factors that may inhibit the achievement of team objectives.

Requirements
  • Registered Mental Health Practitioner/Nurse on NMC or Social Work England Register.
  • Conforms to NMC/Social work England requirements.
  • Experience of working with people experiencing mental health-related difficulties.
  • Ability to assess, plan, coordinate, and manage people with mental health problems in a community setting.
Skills and Knowledge
  • Skills of assessing, planning, treating, and evaluating care for people with mental health-related difficulties.
  • Knowledge of Mental Health Act, the Children Act, Gillick Competency, Human Rights Act, and Mental Capacity Act.
  • Knowledge of pharmacological interventions prescribed in mental health.
  • Knowledge of mental health evidenced-based interventions for children and families.
