As an INT Coordinator, you will play a vital role in a multidisciplinary setting, adapting to evolving priorities and challenges.

Key Responsibilities:

  • Act as a central liaison between your designated Primary Care Network (PCN) and the INT multi-disciplinary team (MDT), ensuring effective communication across various sectors including primary care, community services, mental health, and the voluntary sector.
  • Collaborate with the INT MDT to manage patient information, supporting triage processes and MDT meetings, while capturing outcomes and evaluations.
  • Engage actively in the decision-making processes of the INT MDT.
