Key Responsibilities
  • Provide clinical leadership and expertise to our multidisciplinary team, ensuring the delivery of excellent patient care and outcomes.
  • Work closely with our PCN Lead ACP and wider team to drive service improvement and quality assurance initiatives.
  • Develop and implement effective strategies to improve patient experience and engagement.
  • Collaborate with other healthcare professionals to ensure seamless care pathways and effective interagency working.
  • Contribute to the development and implementation of our quality agenda, ensuring that our services meet the highest standards of care.
Requirements
  • Master's level qualification in Advanced Clinical Practice.
  • Independent prescriber qualification or working towards.
  • Significant recent experience in a senior nursing or allied health professional role within a primary care or emergency setting.
  • Experience of leading clinical teams and professional agendas.
  • Proven track record in improving patient experience and gaining patient/public views.
  • Recent experience of direct involvement in delivering the quality agenda.
