Key Responsibilities- Work as part of the Community Nursing team to provide nursing care to patients in their own homes or community settings.
- Assess care needs and deliver planned care under the direction of the Registered Nurse.
- Contribute to the development of care plans and work collaboratively with other healthcare professionals to ensure seamless care delivery.
- Provide personal care, promote nutritional needs, and assist in the recovery and recuperation of patients.
- Work under the direction of the Nurse (Community) and Nurse Specialist (Community) using your skills and knowledge.
- Level 3 Diploma/NVQ 3 in Health and Social Care or an equivalent qualification.
- Experience of working with older people in a community setting (desirable).
- Competent with IT, including electronic recording.
- Ability to work independently and as part of a team.
