We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our Primary Care team in the West Midlands Regional area within HMP settings. This is an exciting opportunity to be part of the country's first Frailty Hub, overseeing HMP Oakwood, HMP Featherstone, and HMP Brinsford.
Key Responsibilities- Develop and deliver a frailty framework, ensuring high-quality care for patients.
- Conduct Comprehensive Geriatric Assessments (CGA) and provide safe, competent expert holistic care.
- Collaborate with interdisciplinary teams to meet patient needs, supporting the delivery of the Frailty framework and providing clinical education and leadership.
- Upskill the local workforce, demonstrating all four pillars of advanced clinical practice: clinical practice, leadership, management, education, and research.
- A competitive annual salary up to £59,000 per annum FTE, depending on experience.

- Support from the wider team.
- Registered Nurse Adult, Mental Health with current NMC registration or Registered Allied Healthcare Professional with current HCPC registration.
- Independent or supplementary prescriber qualification.
- MSc in Advanced Practice or relevant practice experience.
- Completion of British Geriatric Society Tier 3 Frailty Identification and assessment or willingness to work towards.
- Specialist interest in care of the older person, comprehensive geriatric assessment, frailty, dementia, or end-of-life care.
- Strong IT skills and IT literacy, including competence in digital platforms for consultation and Outlook and SystmOne.
