We are seeking an experienced Advanced Clinical Practitioner to join our Clinical Hub team within our West Midlands Region. As a key member of our team, you will have the opportunity to bring your advanced nursing skills to an organisation dedicated to making a difference to people's lives on a daily basis.

About the Role

In this role, you will be responsible for providing clinical leadership for the mental health team, working collaboratively with interdisciplinary teams across sites to support the delivery of the Frailty framework. You will also provide clinical education, leadership, and upskilling of the local workforce.

Key Responsibilities
  • Complete Comprehensive Geriatric Assessments (CGA), diagnosis, treatment, and evaluation of care.
  • Communicate and work collaboratively with the interdisciplinary teams across sites to support the delivery of the Frailty framework.
  • Provide clinical education, leadership, and upskilling of the local workforce.

Qualifications and Experience

We are looking for candidates with:

  • Registered Nurse Adult, Mental Health with current NMC registration / Registered Allied Healthcare Professional with current HCPC registration
  • Independent / supplementary prescriber qualification
  • MSc in Advanced Practice / relevant practice experience
  • Completion of British Geriatric Society Tier 3 Frailty or willing to work towards
