Main Responsibilities- Lead the community nursing team to deliver high-quality care to patients with specialist palliative care needs.
- Develop and implement policies and procedures to ensure uniformity of nursing care across all community services.
- Manage and develop the skills and competencies of the community nursing team.
- Collaborate with other departments to ensure equity of access to nursing care for all patients.
- Provide leadership and guidance to the community nursing team to promote a culture of excellence and continuous improvement.

We are seeking a highly experienced and skilled individual with a strong background in community nursing and leadership. The ideal candidate will have:
- A first-level registered nurse qualification and a first degree in a relevant subject.
- Appropriate post-basic qualifications in end-of-life/palliative care.
- Experience of successfully managing people as a senior post-holder and working within a multi-disciplinary team.
- Excellent verbal and written communication skills and positive leadership skills.
