About the Role

We are seeking a highly skilled and motivated pharmacist to join our team as a Rotational Pharmacist - Clinical Medicines Management. This is an exciting opportunity to contribute to the development of our clinical pharmacy services and work closely with multidisciplinary teams to improve patient care.

Key Responsibilities
  • Assist with medicines management services across various hospital sites, including Cavell Centre Mental Health Hospital, North Cambridgeshire Hospital, and Intermediate Care Unit Community Hospital.
  • Optimise patient care by participating in medicines management schemes, including medicines reconciliation, self-administration, and medicines supply.
  • Counsel and educate service users about their medicines and provide information and advice to service users, carers, and other healthcare professionals.
  • Active involvement in multidisciplinary team meetings to identify medication problems and provide advice.
  • Support and review prescribing and monitor medicines use and treatment outcomes.
  • Contribute to the education and training of medicines matters for CPFT staff, external stakeholders, and service users.
Requirements
  • Mature and non-judgemental attitude.
  • Flexible and adaptable.
  • Self-motivated.
  • Excellent communication skills.
  • Digital literacy skills for professional business practice.
  • Experience with use of clinical systems that support pharmacy practice.
  • GPhC pharmacist registration.
