Pharmacist - Clinical Medicines Management

We are seeking an enthusiastic and motivated pharmacist to join our team as a Rotational Pharmacist. This is a development post, offering the opportunity to gain experience in clinical medicines management, medicines optimisation, and patient-centred care.
Key Responsibilities- Assist with medicines management services across three pharmacy disciplines, including Cavell Centre Mental Health Hospital, North Cambridgeshire Hospital, and Intermediate Care Unit Community Hospital.
- Optimise patient care by participating in medicines management schemes, including patients' own drugs, self-administration, medicines reconciliation, and medicines supply.
- Counsel and educate service users about their medicines and provide information and advice to service users, carers, and other health professionals.
- Active involvement in multidisciplinary team meetings to identify medication problems, resolve transfer of care issues, and provide advice.
- Support and review prescribing and monitor medicines use and outcomes of treatment.
- Support the education and training of medicines matters for CPFT staff, external stakeholders, and service users.
- Mature and non-judgemental attitude.
- Flexible and adaptable.
- Self-motivated.
- Excellent communication skills.
- Digital literacy skills for professional business practice.
- Experience with use of clinical systems that support pharmacy practice.
- GPhC pharmacist registration.
