Additional Information:

Role Overview:
As a Supported Housing Support Worker, your primary responsibility is to deliver person-centred, high-quality support and housing management to individuals experiencing homelessness with low to medium-level needs. Your goal is to assist clients in making sustainable progress towards increased independence within defined timeframes.
Key Responsibilities:
- Manage referrals into the service following the assessment process.
- Implement and monitor client support plans and risk assessments.
- Ensure compliance with all safeguarding procedures and service policies.
- Provide guidance, information, and enable clients to access appropriate services.
- Maintain daily health and safety standards and ensure the safe operation of the service, including coordination with external contractors.
Qualifications / Requirements:
- Experience working with vulnerable adults and/or individuals with complex needs.
- Ability to establish effective relationships with clients and services to foster pathways for positive change.
- A valid UK driving licence and access to a personal vehicle is required, with business insurance for roles involving travel.
