Position Overview:

Service Care Solutions is looking for a dedicated Evening Care Assistant to deliver a thorough accommodation-based support service to at-risk adults and youth residing in various hostels.

Key Responsibilities:

  • Conduct regular evaluations and reviews of individual needs on a casework basis.
  • Facilitate key work sessions and formulate support plans to effectively identify and enable service users to access necessary assistance from partners, including health services, substance abuse programs, and employment training.
  • Provide a high-quality, responsive support service aimed at helping service users enhance their personal and social skills, maintain stable housing, and improve overall health and wellbeing.
  • Assess and document risks associated with service users, ensuring compliance with safeguarding policies and procedures.
  • Guide service users through the Pathway model, promoting housing alternatives such as private rentals and shared living arrangements.

Qualifications:

  • Experience working with vulnerable populations, including adults and youth with support needs.
  • NVQ Level 3 in Health and Social Care or an equivalent qualification.
  • Possession of a full UK driving license and access to a vehicle.
  • Understanding of safeguarding practices and risk assessment protocols.
  • Familiarity with housing options and benefits.

Contract Type: Initially 3 months
