Role Summary
We are in search of a proactive and compassionate individual with a strong interest in the criminal justice field and substance abuse support services.
The Community Transition Worker plays a vital role in facilitating in-reach services within correctional facilities, aiding individuals in their successful reintegration into society through access to drug and alcohol support services. You will serve as a crucial resource for those re-entering the community, assisting them in attending essential appointments and managing medication collections. Collaboration with a network of professionals will be key to maintaining engagement and progression in their release plans. You will be part of a larger criminal justice team equipped with extensive knowledge and skills, supported by a buddy system for your onboarding process.
Although this is a fixed-term role due to government funding, we encourage you to consider this as a valuable stepping stone within the organization, with opportunities for internal applications for future positions.
A Typical Day as a Community Transition Worker
Each day presents unique challenges and rewards as I work within both prison and community settings, supporting clients in their preparation for release and helping them achieve their personal goals. The release day can be particularly stressful, and clients often express gratitude for having someone to guide them through the process. On release day, I assist clients in meeting with various professionals, navigating benefit claims, registering with healthcare providers, and even shopping for necessities. Following their release, I engage with individuals intensively for up to 16 weeks, focusing on their recovery and connecting them with additional support services. This role is incredibly fulfilling and offers significant job satisfaction.
