We are seeking a highly skilled and compassionate Senior Healthcare Support Worker to join our Community Nursing team. As a key member of our team, you will provide high-quality nursing care to patients in their own homes or community settings.
Key Responsibilities- Work under the direction of the Nurse (Community) and Nurse Specialist (Community) to provide nursing care to patients in the community.
- Contribute to the assessment of care needs and the delivery of planned care under delegation and direction of the Registered Nurse.
- Work collaboratively with other healthcare professionals to provide comprehensive care to patients.
- Develop and maintain strong relationships with patients, families, and carers to ensure person-centred care.
- Participate in the development and implementation of care plans to meet the individual needs of patients.
- Level 3 Diploma/NVQ 3 in Health and Social Care or equivalent qualification.
- Experience of working with older people in a community setting (desirable).
- Competent in the use of IT systems, including electronic recording.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- A comprehensive induction programme, including support to complete the National Care Certificate.
- Ongoing training and development opportunities to enhance your skills and knowledge.
