Our team is dedicated to delivering high-quality care and support to patients in the community, outpatient clinics, and hospital settings.
Main DutiesThis role will involve working predominantly in the community across West Kent, with occasional coverage of pulmonary rehabilitation classes. The successful candidate will support patients experiencing acute exacerbations of COPD, focusing on admission avoidance, complex case management, and oxygen and NIV therapy. As part of our multi-disciplinary team, you will work closely with Respiratory Nurse Specialists, respiratory physiotherapists, and other healthcare professionals to deliver seamless care.
Key Responsibilities- Support patients with COPD and other chronic diseases in the community and hospital settings
- Work collaboratively with the respiratory team to deliver evidence-based care and support
- Develop and maintain strong relationships with patients, families, and healthcare professionals
- Contribute to the development and implementation of care plans and interventions
- Participate in pulmonary rehabilitation classes and other team activities
To be successful in this role, you will need:
- A Foundation Degree in Health and Social Care or equivalent qualification
- A minimum of 3 years' experience as an AHP Support Worker Band 3
- Additional practical and theoretical knowledge of patient conditions and treatment within the relevant specialty
- Excellent interpersonal and communication skills
- Ability to work flexibly and prioritize tasks effectively
