Key Responsibilities

As a Community Nursing Specialist, you will:

  • Provide high-quality care within patients' homes, focusing on rehabilitation, self-management, and promoting independence throughout the care continuum.
  • Collaborate with a team of professionals to support evidence-based practices and foster staff learning and development.

The primary responsibilities of this role include:

  • Diabetes management
  • Wound care, including diagnosis and treatment of leg ulcers
  • Palliative care in close partnership with local hospices and general practitioners
  • Bladder and bowel management, including catheter care
  • Working with a diverse patient group, including those with severe frailty, dementia, end-of-life care needs, complex conditions, and care home residents.

Job Requirements

To excel in this role, candidates should possess:

  • A registered nurse qualification (degree or diploma)
  • ENB 998 or equivalent, or a willingness to work towards it

Experience

  • Experience in a community healthcare setting

Skills and Knowledge

  • Strong numeracy and literacy skills
  • Effective communication and negotiation abilities
  • Competency in various clinical skills, including wound management, catheterization, and palliative care
  • Ability to apply research-based practices
  • Familiarity with local policies regarding community health and social care services
  • A clear understanding of the Community Nursing role
  • Promotion of independence and choice in patient care
  • IT literacy or a commitment to achieve it within an agreed timeframe
  • Knowledge of clinical governance and professional codes of conduct
  • Ability to recognize the boundaries of the role
  • Experience as a practice supervisor
