Job Title: District Nurse / Community Sister/Charge Nurse

We are seeking a skilled and compassionate District Nurse / Community Sister/Charge Nurse to join our PCN Community Team on a 9-month Fixed Term contract. As a key member of our team, you will work in partnership with individuals and their carers to develop personalized plans of care, proactively monitoring and anticipating their changing needs. You will be responsible for managing a designated caseload and delegating the delivery of care by the integrated team.

Key Responsibilities:
  • To apply advanced assessment of physical, mental, and social care needs to provide clinical care and health-promoting interventions that help avoid unnecessary hospital admissions.
  • To develop and analyze audit, interpret data, and use findings to enhance clinical practice.
