Key Responsibilities

The Pharmaceutical Care Specialist will be responsible for the following:

- Conducting thorough medication assessments through in-person, virtual, and electronic consultations, while actively managing patients, including those with intricate medication regimens.

- Providing patient education, prescribing medications, and monitoring treatment plans in accordance with established guidelines and professional standards.

- Collaborating with the broader healthcare team to offer guidance and support on medication management matters.

- Performing medication audits to ensure compliance with best practices.

- Promoting cost-effective prescribing practices within the organization.

- Ensuring that medications are prescribed and monitored in line with the latest clinical guidelines.

- Adhering to safety protocols for medication management and conducting regular safety evaluations.

- Sharing insights and knowledge with colleagues to enhance team performance.

- Participating in regular team meetings and representing the organization at external events.

- Contributing to the achievement of quality improvement targets.

- Conducting medication reviews for patients in care facilities as necessary.

- Actively engaging in initiatives that support organizational goals.

Responsibilities in Detail

- Perform structured medication evaluations for patients with complex medication needs, implementing appropriate prescribing decisions and coordinating necessary tests.

- Reconcile medications following hospital discharges and collaborate with patients and community pharmacists to ensure continuity of care.

- Support long-term condition management through comprehensive reviews.

- Conduct medication assessments in care facilities and adjust prescriptions as needed.

- Contribute to reducing medication-related hospital admissions by empowering patients to achieve optimal outcomes from their treatments.

- Identify and resolve medication-related challenges.

- Serve as a liaison between community and hospital healthcare professionals.

- Address medication inquiries from patients and healthcare providers.

- Collaborate with management to investigate any medication-related complaints.

- Enhance patient safety and quality of prescribing through audits and quality improvement initiatives.

- Implement alerts and notifications regarding medication safety.

- Work with healthcare professionals and patients to apply evidence-based guidelines.

- Participate in multidisciplinary reviews for patients with multiple health conditions.

- Provide training and information on medication-related topics to practice staff.

- Act as a resource for medication information for the entire healthcare team and patients, including details on supplies, dosages, and potential side effects.

Qualifications

Required

  • Licensed Pharmacist

Preferred

  • Independent Prescriber or in the process of obtaining this qualification. Knowledge gained through postgraduate education or equivalent training.

Experience

Required

  • Minimum of 2 years of post-qualification experience, with a solid understanding of primary care prescribing and strategies for enhancing medication management.
  • Strong interpersonal, negotiation, and communication skills, with the ability to convey complex information clearly to diverse audiences.

Preferred

  • Experience working within a GP practice or Primary Care Network.
