About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in West Wick. As a Support Worker, you will play a vital role in enabling individuals to live fulfilling lives, promoting their independence, and supporting them in achieving their goals.

Key Responsibilities

  • Empower individuals to make informed decisions about their lives and support them in achieving their aspirations.

  • Provide support with daily living skills, such as personal care, medication management, and household tasks.
  • Facilitate access to community resources, social activities, and educational opportunities to promote socialization and personal growth.
  • Communicate effectively with individuals, their families, and external professionals to ensure seamless care coordination.
  • Maintain accurate records and contribute to the development of care plans.
