About the Role:
IntraHealth Ltd is seeking an experienced Advanced Nurse Practitioner to join our Primary Care team. As an Advanced Nurse Practitioner, you will be responsible for providing high-quality patient care, working autonomously and as part of a team to deliver evidence-based care.
Key Responsibilities:- Clinical Practice:
- Assess, diagnose, plan, implement and evaluate treatment/interventions.

- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Person Specification:
- Qualifications:
