About the Role

We are seeking a highly skilled and dedicated Qualified Practitioner to join our Learning Disability team at Lincolnshire County Council. As a key member of our team, you will play a vital role in supporting individuals with learning disabilities and their families.

Main Responsibilities
  • Manage a caseload of clients with learning disabilities, providing support and guidance to ensure their needs are met.
  • Review and complete Deprivation of Liberty (DOL) applications, working closely with key stakeholders to ensure the best interests of the individual are upheld.
  • Develop and maintain effective relationships with families, advocacy services, and other professionals to ensure seamless support and care.
  • Work collaboratively with our Learning Disability case management teams, Lincolnshire County Council legal department, and other service providers to ensure the highest standards of care and support.
  • Contribute to the development and implementation of care plans, ensuring they are tailored to the individual's needs and preferences.
About You

We are looking for a qualified and experienced practitioner who is passionate about delivering high-quality care and support to individuals with learning disabilities. You will need to hold a Social Work or Nursing qualification and be registered with the relevant professional body.

Key skills and qualities required include:

  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and professionals.
  • Strong problem-solving and analytical skills, with the ability to think creatively and develop innovative solutions.
  • Ability to work autonomously and as part of a team, with a high level of flexibility and adaptability.
  • Strong understanding of the Deprivation of Liberty (DOL) process and the ability to navigate complex legal frameworks.
