An exciting opportunity is available for a Registered General Nurse, qualified Physiotherapist or Occupational Therapist, to work for Lincolnshire Community Health Services (LCHS). The Assertive In-Reach Team (AIR) is part of the Integrated Urgent & Emergency Care Division of LCHS and based within the acute hospital. The team cover mainly admission avoidance areas - A&E, UTC, SDEC, Fracture Clinic and work closely with our other community services and stakeholders based within the community and acute. The AIR team is pivotal in preventing admission to the hospital ensuring patient needs can be safely met back in the community with the emphasis on home first.

- PREVIOUS APPLICANTS NEED NOT APPLY- To work as an accountable and autonomous practitioner. Recognising and responding to requests for patient reviews in a timely manner enabling a rapid continuation of flow back to the community, to avoid admission to hospital. As well as pro-actively identifying potential patients for admission avoidance, working to the "homefirst" principle. To be able to work alone and also as part of a MDT ensuring the patient stays centre of any decision and plan. Ability to liaise with other key stakeholders and services to provide the most appropriate support/onward needs of the patient.
Ability to think outside of the "box", have influencing skills and ability to lead discussions. To demonstrate well informed clinical decision making in order to provide support and advice to patients, carers and other members of the multidisciplinary teams.
Prepared to work flexibly to meet the needs of the service and respond in a timely responsive way.

Act as a senior specialist advisor for the service
Provide senior leadership, ensuring the highest standard of care, supported by evidence based practice.
Be able to evaluate risk, complex needs and make sound and effective
decisions alone and as part of the multi-disciplinary team
Make professionally autonomous decisions for which you are accountable
for, using well-informed decision making and problem solving skills. Undertake medicine management review for patients
