We are seeking a highly skilled and experienced Senior Elderly Care Practitioner to join our team at NHS Jobs. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the development of our Elderly Care services.
Key Responsibilities- Develop and implement the Scope of Nursing Practice for our Elderly population, ensuring that patient care is delivered in line with Trust guidelines and protocols.
- Provide specialist and advanced clinical skills support to medical staff and the nursing team, ensuring that patients receive comprehensive and coordinated care.
- Triage patients, prioritizing clinical need and initiating treatment plans in accordance with Trust guidelines and protocols.
- Work autonomously to assess individual patient needs, initiate investigations, and determine differential diagnoses, ensuring that patients receive evidence-based treatment and care.
- Coordinate clinical and support services, maintaining or improving the service to meet the needs of our patients.
- Role model compassionate and inclusive leadership, shaping the creation of a collective leadership culture within the Trust.
- Proven experience in Elderly Care, with a strong understanding of the complexities of caring for older adults.
- Advanced clinical skills and knowledge, with the ability to provide specialist support to medical staff and the nursing team.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
- Ability to work autonomously, prioritizing clinical need and making decisions in accordance with Trust guidelines and protocols.
- Strong leadership skills, with the ability to role model compassionate and inclusive leadership and shape the creation of a collective leadership culture within the Trust.
