Job Title: Team Leader

We are seeking an experienced and passionate Team Leader to join our team in Conwy. As a Team Leader, you will be responsible for planning and organizing the delivery of support and care to individuals living in a Supported Living Service.

The Role:
  • Lead a team of support workers to deliver high-quality care and support to individuals with learning disabilities.
  • Develop and implement care plans that meet the unique needs of each individual.
  • Collaborate with other teams to ensure seamless delivery of services.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
The Benefits:
  • Competitive salary of £23,660 (£13 per hour, 35 hours per week)
  • Circa £3000 - £4500 paid on-top
  • Sleep-in's paid at £60
  • On-call £10 per weekday evening, £20 per Sat/Sun
  • 25 days leave plus bank holiday plus an increase of 3 days after 5 years
