Core Duties

The Senior Clinical Practitioner will be responsible for:

  • Practicing in accordance with professional conduct standards and within their scope of practice, being accountable for their clinical decisions and actions
  • Demonstrating a comprehensive understanding of their expanded responsibilities and the limits of their expertise, especially in complex situations
  • Utilizing professional judgment to determine when to seek assistance, reflecting critically on their practice and being open to change
  • Collaborating with patients, families, and caregivers using various assessment techniques
  • Exhibiting effective communication skills to support patients in decision-making and care planning
  • Employing clinical reasoning to inform interventions
  • Initiating, assessing, and adjusting treatment plans, which may include prescribing medications and providing lifestyle advice
  • Managing patients with a variety of acute and chronic conditions, offering expert guidance
  • Implementing and reviewing specialized treatment plans for chronic disease management
  • Identifying and supporting patients at risk of developing long-term conditions
  • Serving as a professional role model within the organization
