Role: Outreach Support Worker

We are seeking a compassionate and driven individual to join our team as an Outreach Support Worker. In this role, you will play a crucial part in helping individuals navigate their way out of homelessness and towards stability.

Responsibilities
  • Provide personalized support to a caseload of clients experiencing homelessness, utilizing assertive outreach techniques to build trust and rapport.
  • Collaborate with various partner agencies, including healthcare providers, substance misuse services, criminal justice organizations, and local authorities, to ensure comprehensive support for clients.
  • Conduct thorough initial assessments to understand client needs and develop individualized support plans that address their unique circumstances.
  • Advocate for clients, assisting them in accessing housing options, navigating benefits systems, and securing essential resources.
  • Offer emotional support during challenging times, providing a safe and empathetic space for individuals to share their experiences and concerns.
  • Facilitate practical skill development, such as managing finances, accessing healthcare services, and maintaining healthy living habits.
  • Maintain accurate records of client interactions, progress, and outcomes to ensure transparency and accountability.
Qualifications

The ideal candidate will possess:

  • Previous experience working with individuals experiencing homelessness or complex needs.
  • A genuine passion for helping others and a commitment to making a positive difference in their lives.
  • Excellent communication, interpersonal, and problem-solving skills.
  • The ability to work independently and as part of a team in a fast-paced environment.
  • A valid driver's license and access to a vehicle.
