About the Role

RDaSH is seeking dedicated and compassionate First Contact Mental Health Practitioners to join our expanding Primary Care Networks (PCNs) across Rotherham, Doncaster, and South Humber. We are committed to providing high-quality mental health care at a local level, ensuring individuals receive the right support in the right place at the right time.

As a First Contact Mental Health Practitioner, you will play a vital role in our integrated mental health services. You will work autonomously within PCNs to screen, triage, and assess patients experiencing mental health difficulties. Your expertise will guide the development of personalized treatment plans and facilitate seamless connections with appropriate secondary mental health services.

Key Responsibilities

  • Provide specialist mental health assessments and interventions within primary care settings.
  • Collaborate effectively with multidisciplinary teams, including GPs, nurses, and social workers.
  • Develop and implement individualized treatment plans based on comprehensive patient needs assessments.
  • Offer expert guidance and support to patients and their families navigating the mental health care system.
  • Contribute to the ongoing development and improvement of mental health services within PCNs.
