Team Leader

We are seeking a highly skilled and experienced Team Leader to join our Patient Flow Hub team at Pennine Care NHS Foundation Trust. The successful candidate will be responsible for leading the team in the areas of admission to hospital, transfers between wards, and assisting clinical teams with informal and formal delayed discharges.
Key Responsibilities- Lead the Patient Flow Hub team in the areas of admission to hospital, transfers between wards, and assisting clinical teams with informal and formal delayed discharges.
- Identify potential complex discharges on admission and collaborate with the multidisciplinary team to set and monitor discharge dates.
- Track and lead on complex discharges to prevent delayed transfers of care.
- Work in collaboration with a dedicated multidisciplinary team, Local authority, and other relevant personnel to plan and manage timely and appropriate transfers of care.
- Be the main source of redirection of admissions in accordance with assessment of need through the identification of the appropriate care setting.

Requirements- RMN or equivalent qualification.
- Extensive experience of working in acute mental health services.
- Sound understanding of current government policies in relation to Acute Care.
- Excellent understanding of the principles of capacity management and bed management procedures.
