Care Team Leader
**About the Role**
As a Care Team Leader at Avery Healthcare, you will play a vital part in delivering exceptional care to our residents. Your focus will be on supporting the home management team in creating a safe, caring, and innovative environment for residents, visitors, and staff.
**Key Responsibilities**
* Lead the person-centred care planning process, implementing, monitoring, and evaluating care plans to ensure the best possible outcomes for residents.
* Assist in the smooth transition of residents from the home, including documentation, medication, and communication with next of kin.
* Monitor the well-being of each resident, identifying any changes that may impact care and ensuring that their needs are met.
* Practice safe systems of work, assessing risks and taking due regard for personal safety and the safety of others.
**Requirements**
* NVQ 3 or equivalent in care for care homes.
* Two years' experience working in a senior position with the resident group.
* Excellent communication skills, both verbal and written.
* Ability to lead staff and act responsibly in the absence of the manager.
