About the Role

We are seeking a skilled and compassionate Mental Health Practitioner to join our team at Devon Partnership NHS Trust. As a key member of our community mental health service, you will play a vital role in providing high-quality mental health services to children and young people.

Key Responsibilities
  • Deliver high-quality mental health services, focusing on assessment, risk planning, and treatment planning for individuals with mental health difficulties and their carers.
  • Impart complex and sensitive information to patients and families/carers regarding assessment, care planning, treatment, and review.
  • Facilitate fair access to social care services and promote social inclusion regardless of age, ethnicity, gender, sexual orientation, or disability.
  • Undertake risk assessments and determine the appropriate course of action, ensuring consultation with senior colleagues as necessary.
  • Develop personalized care packages based on bio-psycho-social assessments and working formulations.

Requirements
  • An appropriate up-to-date professional mental health qualification, such as RMN, OT, or Social Worker.
  • Training to level 2 in Safeguarding children and young people.
  • Able to work 24-hour shift patterns, including nights, in line with service requirements.
