About the Role
We are seeking an experienced mental health nurse or occupational therapist to join our Assertive Outreach Team in Leeds. As a key member of the team, you will provide a city-wide service for patients with complex mental health needs, working closely with other healthcare professionals to deliver high-quality care.
Key Responsibilities
- Provide day-to-day coordination of the team, as required
- Act as care coordinator, responsible for managing a caseload and developing individual care plans
- Work collaboratively with other community teams and agencies to ensure effective care delivery
- Contribute to the development and implementation of innovative care approaches
Requirements
- Registered mental health nurse or occupational therapist with relevant experience
- Significant post-registration experience of working with patients with mental health difficulties
- Ability to work independently and as part of a multidisciplinary team
- Valid driving license and access to a car
