Mental Health Nurse/Occupational Therapist

About the Role
We are seeking an experienced Mental Health Nurse or Occupational Therapist to join our Assertive Outreach Team in Leeds. As a key member of the team, you will provide a city-wide service for patients with complex mental health needs, working closely with other healthcare professionals to deliver high-quality care.
Key Responsibilities
- Provide day-to-day coordination of the team, as required
- Act as care coordinator, responsible for managing a caseload and developing individual care packages
- Work collaboratively with other community teams and agencies to ensure effective care delivery
- Contribute to the development and implementation of care plans, ensuring they meet the needs of service users
- Provide education and support to students and other staff, promoting a culture of learning and development
Requirements
- Registered Mental Health Nurse or Occupational Therapist with the NMC or HCPC
- Significant post-registration experience working with service users who experience psychosis
- Valid driving license and access to a car
