Job Summary

We are seeking a highly skilled and motivated Clinical Case Manager to join our team at Childrens Complex Care. As a key member of our healthcare team, you will be responsible for managing a caseload of children and young people with complex healthcare needs.

Key Responsibilities
  • Clinical Case Management: Safely manage a caseload of children and young people with complex healthcare needs in the community setting.
  • Assessment and Planning: Complete comprehensive assessments and develop bespoke care delivery plans to meet the unique needs of each child or young person.
  • Clinical Governance and Risk Management: Ensure that nursing practice is in accordance with the NMC Codes of Conduct and that all reasonable measures are in place to deliver safe and effective evidence-based care.
  • Education and Training: Provide classroom-based training and support to community staff, including competency assessments for non-registered healthcare staff on clinical delegated tasks.
  • Leadership and Collaboration: Work collaboratively with the Education Team, ICBs, and external Multi-Disciplinary Teams to ensure high-quality care delivery and effective engagement.
Requirements
  • UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills.
  • Experience in managing complex healthcare needs in a community setting.
  • Ability to work collaboratively with a multidisciplinary team.
  • Strong leadership and training skills.
