Key Responsibilities- Undertake key-working responsibilities for a caseload assigned by the Team Leader/Manager
- Conduct initial and continuous assessments of needs and potential risks, agreeing levels of support and actions
- Create support/action plans for providing appropriate services based on the assessment and reflecting available services and resources
- Build trusting relationships with customers, working proactively with the team to handle the service caseload and administrative responsibilities
- Support customers in undertaking domestic tasks, providing practical assistance where necessary, to ensure a high quality of accommodation
- Develop links with local statutory and voluntary organisations to provide engagement opportunities for customers in the community
- Empower customers to access the services and benefits they are entitled to
- Undertake administrative work, maintaining accurate and comprehensive tenant/customer records to professional standards
- Encourage and enable tenants to manage their rent accounts effectively in conjunction with the Housing Management team
- Proactively chase referrals to ensure properties are filled efficiently
- Engage in learning and development activities to increase knowledge and skills
- Provide day-to-day instruction/supervision of Assistant Support Workers/domestic staff/Personal Support Assistants where appropriate
- Undertake other duties consistent with the grade and nature of the post as may be reasonably assigned
- Involving customers in the design, development, and delivery of the service
- Ensuring Look Ahead Health and Safety policies are adhered to at all times and upholding all health and safety responsibilities within relevant policies and local protocols
