Mental Health Practitioner

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team in Primary Care. As a key member of our General Practice team, you will play a vital role in delivering high-quality mental health services to our patients.
Key Responsibilities- Contribute to the development and delivery of our primary care mental health service model, working closely with colleagues in specialist community mental health services.
- Provide mental health assessments and interventions in Primary Care, working with patients and their families to address issues such as housing, debt, carer fatigue, and addictions.
- Work closely with social prescribers and recovery navigators to support adults with mental health needs, whose difficulties are best understood within a biopsychosocial model.
- Facilitate seamless transitions between teams and services, ensuring that patients receive the right level of care and support.
- Connect people to appropriate community and voluntary sector support, working alongside Social Prescribers.
- Provide targeted formal and informal learning opportunities for all staff within the primary care teams to develop their knowledge and capabilities within the mental health component of holistic care.
- Professional qualification in mental health equivalent to Registered RMN, Registered Social worker or Registered Therapist.
- Current registration with a professional body and commitment to CPD.
- Significant skills and experience in educating and training the mainstream workforce, particularly non-specialist mental health workers.
- Demonstrates substantial experience of assessing risk and developing risk management strategies.
