Clinical Case Management Coordinator - Weekend Availability Required
This is a remarkable opportunity to become part of our dedicated team, committed to delivering safe patient care in their residences, thereby preventing unnecessary hospital admissions and facilitating timely discharges.
Our Urgent Community Response (UCR) and Virtual Ward (VW) teams operate collaboratively to provide a rapid two-hour response to patients facing urgent health crises in their homes. We utilize advanced technology to monitor patients remotely, supported by our Digital Health team.
Key Responsibilities
The successful candidate will oversee the clinical caseload of our UCR and VW teams. Responsibilities include:
- Maintaining and updating the team’s clinical caseload.
- Collaborating with the Multi-Disciplinary Team to enhance service effectiveness.
- Managing all administrative tasks associated with the clinical caseload.
- Utilizing various patient information management systems (such as EMIS, Docobo, Patient Centre) to input necessary data.
- Handling sensitive information with discretion and professionalism.
- Monitoring the distribution and collection of equipment used for home patient monitoring.
- Maintaining an accurate inventory of home monitoring devices.
- Collecting data based on clinical outcomes.
- Participating in and organizing team audits as needed.
