We are seeking a Support Practitioner to join our team in Perthshire, providing individualised support to adults with complex needs who are experiencing or at risk of homelessness.
Key Responsibilities- Deliver 1-2-1 support to individuals in their own homes, enabling them to develop skills required to live independently.
- Assist with budgeting, finances, and day-to-day household activities.
- Help individuals access appropriate supports from other agencies.
- Keep accurate notes regarding support visits and complete risk assessments, support plans, and reviews.
- No previous working experience is needed; full training will be provided.
- IT skills are required for all our vacancies.
- Successful candidates will be required to register with the SSSC within 6 months of start date.
Our Perth Floating Housing Support Service provides support to adults with varying needs within their own homes. We aim to enable individuals to develop the skills required to independently maintain and sustain their tenancies.
