About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Cornerstone Community Care. As a Support Practitioner, you will play a vital role in providing support to adults with various disabilities and/or additional support needs in Perth.

Key Responsibilities
  • Provide personal care, shopping, finances, cooking, housework, and medication support to individuals
  • Participate in and support individuals' hobbies and interests, such as work experience on a farm, activity clubs, swimming, football, boccia, day trips, and eating out
  • Work collaboratively with the team to provide tailored support to each individual
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders
Requirements
  • Confidence to work independently and as part of a team
  • A caring and compassionate attitude
  • Excellent communication and organisational skills
  • A positive attitude with a willingness to learn and develop new skills
  • Good administration skills and ability to use computers
Working Arrangements

Shift patterns will usually involve working split shifts during the hours of 0800-2000 hours and will include weekend working on a rotational basis. There may be occasions where you work later shifts to support individuals to attend shows, concerts, and activity groups, and sleepovers to support trips away.
