Rehabilitation Case Manager Opportunity

We are seeking an experienced Rehabilitation Case Manager to join our team at Active Care Group. As a Rehabilitation Case Manager, you will play a vital role in supporting adults and children with ongoing needs to improve their quality of life and maximize their potential.

About the Role

This is a fantastic opportunity to work with a dynamic and supportive team, providing individualized solutions for people affected by injury or illness. You will be responsible for advocating for clients' needs, building strong relationships with families, deputies, and litigation solicitors, and implementing individualized Support & Rehabilitation Plans.

Key Responsibilities
  • Advocate for clients' needs and build strong relationships with families, deputies, and litigation solicitors
  • Produce action plans and goals with timeframes that are specific and relevant to the client
  • Recruit, select, and manage a team of care/support workers for clients
  • Implement individualized Support & Rehabilitation Plans with specific guidelines for the support team
  • Monitor and review Case Management Plans and Support & Rehabilitation Plans, amending as per the client's needs
  • Provide a flexible working pattern to the support team, ensuring the smooth operation of service, including working out of core hours, weekends, and public holidays
  • Compile risk assessments on key activities as required and identify risk management strategies to manage the safety of all parties in a positive manner
Requirements
  • Hold a UK professional health qualification requiring HCPC, NMC, or Social Worker registration
  • Full driving license and access to a car
  • Previous experience as a Case Manager, supporting adults or children
  • Thorough knowledge of CQC fundamental standards and key lines of enquiry
  • Full working knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS), and Safeguarding frameworks
  • Ability to address and respond to employee issues in a fair and consistent manner
  • Strong organizational, time management, and planning skills
  • Flexible, professional, and positive approach to multi-site working
