About the Role

The Social Worker position at Cardiff County Council is a challenging and rewarding opportunity to make a positive impact on the lives of families and individuals in need of support.

As a Social Worker, you will be responsible for undertaking assessments, care planning, and person-centered planning to ensure that individuals receive the necessary support to meet their care needs.

You will work closely with partners to identify and meet individuals' care support needs, and will be expected to have excellent assessment and communication skills, as well as the ability to write concise records and work as part of a team.

Key Responsibilities
  • Undertake assessments and care planning to ensure that individuals receive the necessary support to meet their care needs
  • Work closely with partners to identify and meet individuals' care support needs
  • Develop and implement person-centered plans to support individuals in achieving their goals
  • Conduct home visits and assessments to gather information and provide support to individuals and families
  • Collaborate with other professionals to ensure that individuals receive a coordinated and effective response to their needs
